mrs. rachel adams, rn, is our school nurse and serves students, faculty and staff by providing care for daily illnesses and injuries, assisting students in the management of chronic health conditions, prepares for health emergencies, oversees the collection, review and storing of student health information, acts as a liaison to community health resources, and promotes overall health and wellness with our students. we are blessed to have her as a resource for our students. below you will find links to policies as well as downloadable forms for all issues concerning student health.

form(s) must be completed at the start of each school year
request for medication administration (complete if medication will be sent to school. this includes epipens. provider signature is required)
allergy and anaphylaxis emergency care plan (complete if your child has a severe allergy. provider signature is required)
Asthma Information & Care Plan (complete if you child has asthma. provider signature is required)
diabetes emergency care plan (provider specific. contact school nurse with any questions. provider signature is required.)
cardiac emergency care plan (complete if your child has a cardiac condition and needs an emergency care plan in place. provider signature is required)
seizure information form and action plan (complete if your child has a history of seizures. provider signature is required.)
other medical alert form (complete if your child has a medical condition that the school needs to be aware of that is not listed above)
student athlete permission form (all students 3rd-5th grade that plan to participate in a sport at bbs. includes information and forms for both concussions and sudden cardiac arrest.)
due to the current covid-19 pandemic, please note a few additional health related items will be needed prior to the start of our fall semester.
thermometer: parents are required to take their child’s temperature each morning prior to arriving on campus.
masks: each student will need to have access to 2 masks. while we hope not to have our students in masks, there might be times when it becomes necessary and we want to be prepared for those times.
